Condition
Management:
ICM provides two levels of case
management – Condition Management (CM) and Large
Case Management (LCM). Patients needing
management for such things as: intravenous
antibiotic therapy for a resolvable infection or
hospice for the patient that has made the
decision not to pursue further cancer treatment
are examples of patients that are served by the
CM program. The Condition Management list is
extensive and includes the management of many
types of illnesses and services.
These are provided by
professional experienced nurses working with the
patient and the care team to insure appropriate
cost-effective care. Our Condition Managers
also provide direct patient education for over
90 conditions, empowering patients to improve
their quality of life.
